Resposibilities:
- Evaluate the patient’s functioning level and recommend enhancements to hearing ability.
- Conducts initial and ongoing comprehensive assessments of patient’s needs.
- Establishes Speech Therapy treatment plan in consultation with physician and revises as necessary.
- Assists in interdisciplinary patient care plan development related to Speech Therapy.
- Ensures appropriateness of physician’s orders and discusses necessary changes.
- Communicates with team members and instructs them in speech pathology techniques.
- Teaches, supervises, and counsels family and patients in Speech Therapy program.
- Evaluates patient, family, and home situation to determine required instruction and assistance.
- Arrange outpatient services for procedures not feasible at home.
- Records evaluation data, treatments, and patient response timely.
- Reports patient’s treatment reactions and changes to physician.
- Participates in developing and revising physician’s treatment plans, keeping informed of patient’s status.
- Communicate with physicians per agency policy regarding patient status changes.
- Evaluate effectiveness of speech pathology intervention and update care plan.
- Document and chart all services rendered.
- Submit ALL clinical notes and progress notes within 24 hours of each care visit.
- Participates in meetings, conferences, and in-service programs.
- Participates in discharge activities and completes discharge summaries.
- Adhere to federal, state, and accreditation requirements.
- Regularly check your schedule to see assigned patients.
- Must maintain patient privacy and confidentiality (HIPPA).
Qualifications:
- Master’s Degree in Speech-Language Pathology preferred, with a Bachelor’s Degree in Speech and Hearing Therapy.
- Licensed in Maryland.
- Minimum of six months institutional/home health experience.
- Knowledge of home health, ST techniques, and documentation.
- Strong organizational and time management skills.
- Ability to communicate effectively with managers and staff.
- Must pass a criminal background check.
- Current CPR certification.
- Effective communication in English is required.
Job Conditions:
- Must have a driver’s license and be able to drive to client residences.
- Ability to access homes, which may not be wheelchair accessible.
- May involve bending, stooping, lifting patient weight, and carrying equipment.
- Physical requirements may involve lifting and carrying patients and equipment.
- Adequate hearing, eyesight, and physical dexterity needed to perform duties and communicate effectively.
- Strong verbal and written communication skills required.
Equipment Operation: Utilizes speech therapy equipment and hand-washing materials.
