Responsibilities:
- Conducts comprehensive assessments of patient needs.
- Performs evaluations, assessments, and routine therapy visits.
- Develops treatment plans in consultation with physicians, revising them as needed.
- Supports physicians in evaluating patients through various tests.
- Administers treatments to relieve pain and restore function using physical methods.
- Consult and educate staff on treatment plans.
- Teaches, supervises, and counsels patients and families in the therapy program.
- Instruct patients on the use of assistive devices.
- Provides necessary equipment for treatment.
- Arrange outpatient services when necessary.
- Supervises Physical Therapy Assistants and paraprofessionals.
- Reports patient reactions to treatment and changes in condition to physicians.
- Collaborates with other team members and instructs them in relevant therapy procedures.
- Document and chart all services rendered.
- Submit ALL clinical notes and progress notes within 24 hours of each care visit.
- Participates in meetings, conferences, and in-service programs.
- Contributes to discharge planning and completes discharge summaries.
- Adhere to federal, state, and accreditation requirements.
- Regularly check the schedule to see assigned patients.
Assists in The Agency Admission Process:
- Obtains medical history relevant to the current condition.
- Evaluates patient and home situation for necessary instruction and assistance.
- Ensures appropriateness of physician orders and discusses changes as needed.
Qualifications:
- Graduate of an AMA-approved Physical Therapy school.
- Licensed in Maryland.
- Minimum of six months institutional/home health experience.
- Knowledge of home health, PT techniques, and documentation.
- Strong organizational and time management skills.
- Ability to communicate effectively with managers and staff.
- Must pass a criminal background check.
- Current CPR certification.
- Effective communication in English is required.
Job Conditions:
- Must have a driver’s license and be able to drive to client residences.
- Ability to access homes, which may not be wheelchair accessible.
- Physical requirements may involve lifting and carrying patients and equipment.
- Adequate hearing, eyesight, and physical dexterity needed to perform duties and communicate effectively.
- Must pass background check
- Strong verbal and written communication skills required.
Equipment Operation: Utilizes physical therapy and medical equipment, along with hand-washing materials.
