License Practical Nurse (LPN)

Responsibilities:

  • Assist the Registered Nurse in preparing, implementing, and evaluating the care plan.
  • Aid in performing specialized procedures and patient care under the direction of the Registered Nurse.
  • Monitor the client’s condition and evaluate the effectiveness of nursing services.
  • Provide teaching to the client, family, and caregivers regarding nursing care and disease process.
  • Foster teamwork and participate in case conferences and in-service meetings.
  • Continually strives to improve nursing skills through education, workshops, etc.
  • Document and chart all services rendered.
  • Submit ALL clinical notes and progress notes within 24 hours of each care visit.
  • Observe and report client symptoms and changes in condition.
  • Adhere to federal, state, and accreditation requirements.
  • Check-in with the appropriate supervisor as needed.
  • Regularly check the schedule to see assigned patients.
  • Notify the Agency of any absences affecting service provision.
  • Must maintain patient privacy and confidentiality (HIPPA).

Treatments and Procedures: Licensed Practical Nurses may perform certain treatments and procedures according to agency policies and state regulations, including medication administration, IV management, and wound care.

Qualifications:

  • Graduation from an accredited School of Practical Nursing.
  • Licensed Practical Nurse in the state of Maryland.
  • Minimum of six months experience in community/home health or hospital setting is preferred.
  • Knowledge of home health care principles, licensed practical nursing techniques, and documentation.
  • Strong organizational and time management skills.
  • Must pass a criminal background check.
  • Current CPR certification.
  • Effective communication in English is required.

Job Conditions:

  • Must have a driver’s license and be able to drive to client residences.
  • Ability to access homes, which may not be wheelchair accessible.
  • Physical requirements include walking, sitting, standing, lifting, and turning clients.
  • Adequate hearing, eyesight, and physical dexterity needed to perform duties and communicate effectively.
  • Strong verbal and written communication skills required.

Equipment Operation:

  • Thermometer, B/P cuff, glucometer, stethoscope, penlight, handwashing materials, and additional basic medical equipment.
