Certified Nursing Assistant (CNA)/Home Health Aide (HHA)

Responsibilities:

  • Adhere to the plan of care to deliver safe and competent service to the client.
  • Assist the client in maintaining personal hygiene and ensuring a healthy, safe environment.
  • Plan and prepare nutritious meals and assist with shopping as needed.
  • Aid the client in ambulation as prescribed by the physician and overseen by the nurse.
  • Support therapy personnel as required during rehabilitative processes.
  • Encourage the client’s independence in alignment with the nursing service plan.
  • Stimulate the client’s mental alertness through engaging activities.
  • Provide emotional and psychological support to the client and household members, building trusting relationships.
  • Report any changes in the client’s condition or home situation to the nurse or Aide supervisor.
  • Perform routine housekeeping tasks to maintain a safe and comfortable environment.
  • Document all services rendered.
  • Submit ALL clinical notes and progress notes within 24 hours of each care visit.
  • Foster teamwork and participate in case conferences and in-service meetings.
  • Regularly check the schedule to see assigned patients.
  • Notify the Agency of any absences affecting service provision.
  • Must maintain patient privacy and confidentiality (HIPPA).

Qualifications:

  • High school diploma or GED preferred.
  • Completion of a nurse aide training and competency evaluation program certified by the Maryland Board of Nursing.
  • Minimum of six months experience in community/home health or hospital setting is preferred.
  • Knowledge of home health care principles, CNA techniques, and documentation.
  • Evidence of personal care, housekeeping, and meal preparation knowledge.
  • Must pass a criminal background check.
  • Current CPR certification.
  • Effective communication in English is required.

Job Conditions:

  • Must have a driver’s license and be able to drive to client residences.
  • Ability to access homes, which may not be wheelchair accessible.
  • Physical requirements include walking, sitting, standing, lifting, and turning clients.
  • Adequate hearing, eyesight, and physical dexterity needed to perform duties and communicate effectively.
  • Strong verbal and written communication skills required.

Equipment Operation:

  • Thermometer, B/P cuff, stethoscope, and handwashing materials.
